An industrial-services contractor working on live coal-terminal and port sites needed more than a set of documents — it needed an HSE function it could rely on week to week: a working management system, credible incident investigation, high-risk task control, and the assurance evidence its principal clients demand.
How we helped
HSEQ Logic works as the organisation's retained HSE partner — an extension of the team rather than a one-off report:
A single integrated business management system: a current policy suite, health-and-safety and environmental management plans, and a live risk/hazard/legal register.
Independent ICAM investigations of workplace incidents, each with an executive-level presentation for leadership.
High-risk task control — job hazard and environmental analyses for blast-and-coat and chemical handling, plus a controlled chemical-introduction process.

The outcome
Investigating incidents across multiple sites, we did what a single investigation never can: we found the pattern. The same root cause — unmapped changes in scaffold-deck levels combined with task fixation during blasting — was recurring across different worksites. Rather than close each incident in isolation, we turned it into a systemic control: formal hazard mapping and physical delineation of scaffold work fronts, built into the risk-assessment templates and pre-task walkdowns used everywhere.
