The challenge
On a Tier-1 iron-ore project, a heavy roll of material was released to the ground during unloading and came to rest inside an exclusion zone. No one was hurt — but the potential was serious, and the contractor chain needed an independent investigation that would stand up to client and regulator scrutiny, not an in-house account of its own event.
How we helped
HSEQ Logic acted as independent lead investigator and ICAM facilitator, running the investigation proportionate to the event's potential:
Full ICAM — structured evidence collection across people, environment, equipment, procedures and organisation; timeline reconstruction; and absent/failed-defence analysis.
An evidence register and index where every finding is referenced to a citable source — the report is auditable end to end.
Review of the loading and unloading procedures, safe-work method statements and site rules against what actually happened.
A clear executive presentation so leadership could see the findings and decisions at a glance.
The outcome
We delivered a defensible, fully evidence-indexed ICAM to executive level. It established four interrelated root causes — load-stability in transport, stability not reassessed before restraints were released, no supervisor hold-point or checklist, and an exclusion-zone breach — each with a targeted corrective action, including a controlled stability checklist, physical exclusion-zone demarcation and reinforcement of stop-work authority, with a 90-day effectiveness review.
Why it matters
When the investigation has to be credible to a Tier-1 client and a regulator, independence and evidence discipline are everything. Every conclusion is traceable to a source — nothing rests on assertion.
